This book focuses on the causes, consequences, and possible means of avoiding organizational accidents. While individual accidents are more frequent and often target the individual for blame, organizational accidents are deep rooted errors in the daily function of organizations that increase the likelihood for disaster. The author discusses the relationship between organizational hazards caused by technology and human factors and possible methods that organizations can use to defend against those hazards. He uses disasters in numerous industries to illustrate how organizational accidents occur and how to prevent them. Lastly, he argues that organizations must provide and cultivate a culture of safety if they are to decrease the possibility of on-site accidents, he provides three models for doing this, and he discusses the consequences for workers and organizations of neglecting safety.
